What look do you want from your breast surgery? And what are your options?
(Hint: Size is just one consideration in breast enlargement surgery.)
Contrary to popular perception, breast augmentation isn't simply about increasing bra size. There are other personal, aesthetic, and medical considerations that go into "designing" the breasts that will please you most.

Dr. Markmann will work closely with you to understand your desired results-and to present the surgical options most likely to produce those results. In one or more in-depth consultations, you and Dr. Markmann will discuss such issues as:

Breast Shape. Depending on the look you want, you can choose from two basic types of breast implants:
1.  Round Implants: These give a round curve in the upper part of the breasts.
2.  Anatomical Implants: These implants, shaped more like a natural breast, provide a gentle slope in the upper breast area.  Anatomical implants require special surgical techniques. Dr. Markmann is one of the few plastic surgeons in the Baltimore area offering these implants.
Q: Why does surgery with anatomical implants require special attention?

Breast Width. The width of your augmented breasts will determine both the outside curve of your breasts and the amount of cleavage you have.

Physical and Medical Considerations in Breast Enlargement

Amount of existing breast tissue. The size of your breast implants may be determined, in part, by how much breast tissue you have now. (If you do not have much existing breast tissue, you may need to limit the size of your implants.)

Overall physical condition. As with planning any type of surgery, a thorough review of your health history and your current health status-recent illnesses, medications, tobacco or alcohol use, vitamins-is essential.

In the majority of cases, Dr. Markmann performs breast augmentation as an outpatient ("same day") procedure. The surgery itself takes only one to two hours, and most patients return home within 1-2 hours of surgery, following a brief rest in a post-operative suite.

Anesthesia
Most patients choose general anesthesia, which seems to provide the greatest degree of comfort in breast augmentation procedures. However, if desired, local anesthesia may be an option for some individuals.

The Incisions
Incision Placement
The surgical incision for inserting implants can be made in one of four places:
1. Under the arm ("transaxillary" incision)
2. Around the nipple ("periareolar" incision)
3. In the fold under the breast ("inframammary" incision).
4.Through the belly button (trans umbilical incision) often referred to as the TUBA or scarless approach.

Not every incision type works well for every woman. Prior to breast surgery, Dr. Markmann will discuss which option(s) can produce the best possible surgical results for you-and leave the least visible incision scars.

Incision Size
The size of your incisions will depend on many factors, including the amount of existing breast tissue you have and the desired size of implant. In general, incisions can range from about 2 to 2.5cm (about 1 inch).

Inserting the Breast Implant
Breast Implant Placement
Implants can be placed in one of two areas in the breast:
1. Directly behind the breast tissue ("subglandular")
2. Underneath the chest muscle ("submuscular")

Dr. Markmann will recommend a placement site depending on such factors as the look you desire, previous breast surgeries, and amount of existing breast tissue.

Breast Implant Infusion
In order to keep the incision as small as possible, the breast implant is empty when the surgeon inserts it into the breast. Once the implant is properly placed, it is filled with saline fluid, with the surgeon adjusting for size and symmetry.

Recovery Period
How will I feel?
For the first few days after surgery, you will most likely feel sore and tired; you will want to stay home and rest. You should be able to return to work within a few days (as long as your job does not require strenuous activity).

It usually takes about month for breast discomfort to resolve. Your breasts may be sensitive to direct physical contact for two or three weeks. And you will need to avoid any strenuous activities for that first month.

How will my breasts look?
Your breasts will go through a short period of healing and adjusting. Over the course of several weeks, swelling will diminish, incision marks will lighten, and the skin will adjust to the new breast size. After about a month, the full results of your breast augmentation will be apparent.

Risks
Breast augmentation, like any surgical procedure, entails some risks. General risks of surgery include reaction to anesthesia, infection and bleeding. Risks specific to breast augmentation include implant deflation, contraction of scar tissue around the implant, and shifting of the implant.

Dr. Markmann will review risk factors and potential complications with you in detail, so that you can make an informed decision about proceeding with the surgery.

Q: Why does surgery with anatomical breast implants require special attention?
Anatomical implants require a good deal of precision on the surgeon's part. Because they are shaped like a natural breast, with a gentle slope at the top and more fullness at the bottom, anatomical implants must be positioned very precisely and securely; any misplacement or shifting of the implant could lead the breast to appear misshapen. Round implants, on the other hand, do not require as much exactitude. By simple fact of their "roundness," they can shift somewhat after surgery yet still maintain the desired breast shape. This is not to imply that surgery with round implants is simple or risk-free; the surgeon must be highly skilled and experienced. But women who want the more "sloping" effect of anatomical implants should choose a surgeon who has a solid track record with this type of implant.

Q: How long is the recovery?
Length of recovery will depend on the individual. Implant size, Position (submuscular vs subglandular position) and the individuals occupation, will greatly affect the time required for recuperation. Time off work may vary from several days up to two or more weeks.

Q:How much does Breast Augmentation surgery cost?
Total cost of breast augmentation surgery by Dr Markmann will vary depending on the implants chosen.

Q: Does that fee include everything?
Yes. This fee includes Dr Markmann's fee, implant cost, Operating room charges and Anesthesia fees as well as all post op follow up visits and yearly check up appointments. Patients frequently will call Doctors offices for estimated prices and ask "How much does Dr. Blank charge for breast augmentation?" The answer is usually in the 2-3 thousand dollar range, which sounds like a great deal. What they do not explain is that this price is the doctors fee only and does not include anesthesia, operating room charges or implants. Be careful.

Q: Is financing available?
